    Student Notice
  • Student Notice: Criminal Background and Drug Tests

  • The preliminary interview with the Admissions Department will include a discussion of criminal background checks and drug testing. It is in your best interest to note the following:

CDE Career Institute is committed to ensuring that students have the ability to benefit from the education received at the school. Clinical facilities affiliated with CDE Career Institute require that students placed at their facility for clinical rotation experiences clear a criminal background check and a drug test before beginning their assignments. Students whose background check reveals criminal history, or who have a positive drug test, may be denied access to the clinical site and, as a result, will not have sufficient clinical experience to complete the program. In addition, the New Jersey Board of Nursing may deny an individual the opportunity to sit for the NCLEX-PN examination or to obtain a license because of criminal history.

All LPN applicants must complete a criminal background check as required by N.J.A.C. 13:37-1.8(h) and a drug screening before the start of clinical rotations. Fully qualified applicants are required to complete the Student Disclosure of Criminal Convictions, Pending Criminal Charges and Certain Disqualifications form before admission.

Applicants who have engaged in any activity or behavior that may be considered abuse, neglect, or exploitation of a minor or of an incapacitated or dependent adult, or who have been convicted of any crime involving fraud or dishonesty, or drugs, or for which imprisonment of one year or more has been imposed, are urged to seek guidance about program completion and licensure requirements from the Director of Admissions before submitting an application.

  • Instructions: Student Disclosure of Criminal Convictions, Pending Criminal Charges and Certain Disqualifications

  • Requirement to Disclose. As part of its admissions process, the school requires you to complete the disclosure form and disclose information regarding any criminal convictions, pending charges, or other disqualifying factors that you may have.

Use of the Information Disclosed. The existence of a conviction, pending charge, or other disqualification does not necessarily mean that you will be denied admission to the school or its programs. Each case is evaluated on its own facts. The nature, severity, and recency of the crime, charge, or disqualification, and its relationship to the interests and needs of the school, clinical partners, and patients, will be assessed, and the responsibility and rehabilitation of the person will be carefully considered and weighed.

Full Disclosure Required. You must complete the form to the best of your ability. Your answers must be truthful, accurate, and complete. If you know of certain information but are unsure whether to disclose it, you must disclose it. The school will determine whether the disclosure was required and, if so, whether the information is material.

Consequence of Failure to Disclose. By signing the form, you certify that you understand and agree that failure to disclose material information may result in immediate exclusion, suspension, or expulsion from the school.

Continuing Duty to Disclose. By signing the form, you also certify that you will notify the school immediately of any inaccuracies in, or corrections to, the information you disclose, and of any new conviction, charge, or disqualification until you have completed the program.

Definitions. For purposes of the form:

    • “Pending criminal charge” means any charge that is currently pending or threatened to be brought against you.
    • “Criminal conviction” means any instance in any state or country where you pleaded no contest or guilty, or were found guilty by a judge or jury, of charges that you committed a crime, other than those excluded below. It includes (1) a conviction you are currently appealing; and (2) a juvenile conviction, an expunged conviction, or a pardoned conviction, unless the law of the state where the juvenile conviction, expungement, or pardon occurred expressly permits you to lawfully withhold its disclosure.
    • “Criminal conviction” does not include, and you need not disclose, any instance where you were: (1) arrested but not charged; (2) arrested and charged but the charges were dropped; (3) arrested and charged but found not guilty; (4) found guilty but the conviction was overturned on appeal; or (5) cited for a traffic offense with a fine of less than $200.
    • “Disqualification” means any occasion that resulted in your removal or exclusion from any health care facility due to an allegation that you engaged in a form of neglect or other misconduct.
  • Disclosure of Criminal Convictions, Pending Criminal Charges and Certain Disqualifications

  • 1. Have you read and understood the Disclosure Instructions?*
  • If no, please do not complete this form until you have done so.

  • 2. Have you ever been convicted of a crime as defined in the instructions?*
  • If yes, explain for each crime.

  • 3. Are criminal charges currently pending or threatened against you?*
  • If yes, explain for each charge:

  • 4. Have you ever been removed or excluded from a health care facility for alleged neglect or misconduct?*

  • LPN Attendance and Satisfactory Academic Progress Policy

Summary of the School Catalog and the LPN Program Academic Standards Addendum
  • This policy applies to all LPN students regardless of funding source. If this summary differs from the School Catalog, the catalog governs.

  • ATTENDANCE

    • Attendance is mandatory. LPN students must maintain a minimum attendance rate of 90% in all scheduled instruction, including theory/classroom, laboratory, and clinical rotations. Students must contact the school on the first day of any absence.
    • Missed scheduled time is recorded as absence in clock hours. All missed theory and clinical hours must be made up.
There is no limit on make-up requests, but make-up sessions are not guaranteed, are subject to instructor, facility, and clinical site availability, and do not excuse the 90% requirement. There is no charge for make-up sessions. Days the school is closed are not scheduled hours. Scheduled hours during an approved leave of absence are not counted as absences; the enrollment period is extended so that all 1,501 clock hours are completed.
    • A student whose attendance falls below 90% in any course is placed on attendance probation, must meet with the Director of Education within five (5) business days to develop a written remediation plan, and may not begin a new clinical rotation until attendance in the affected course is restored to at least 90%. Continued non-compliance may result in course failure, removal from clinical rotations, and administrative withdrawal.
    • After 14 consecutive calendar days of absence without an approved leave of absence, a student who is not in
documented two-way contact with the school is subject to administrative withdrawal and is billed under the Enrollment Agreement and the Cancellation and Refund Policy.

    GRADING AND PROGRESSION

    • Minimum grade of 78% (C+) in every nursing theory course and a grade of Pass in every clinical practicum course; minimum cumulative GPA of 2.5.
    • Students must complete all courses in the current term before advancing to the next term. Failure of two (2) or more nursing courses (theory or clinical) results in dismissal. Students must meet the 90% attendance standard and maintain the 78% (C+) standard to begin or continue clinical rotations.
  • SATISFACTORY ACADEMIC PROGRESS (SAP)

    • Financial Aid Warning: a student who was meeting SAP at the prior evaluation (or is at the first evaluation at 450 clock hours) and does not meet SAP is placed on warning for the next payment period and remains eligible for federal aid during that period. Warning is not granted for two consecutive payment periods.
    • Appeal and Financial Aid Probation: a student who still does not meet SAP after the warning period loses federal aid eligibility, but may file a written appeal with the School Director within five (5) days of the written notice, based on the death of a relative, an injury or illness of the student, or other special circumstances, explaining why SAP was not met and what has changed. If the school approves the appeal in writing and the student signs an academic plan, the student is placed on Financial Aid Probation for one payment period.
    • A student who is not meeting SAP must also meet with the Director of Education within five (5) business days to develop an academic improvement plan. Attendance probation is separate from Financial Aid Warning and Probation.
    • Leave of absence: requested in advance (unless unforeseen circumstances prevent it), in writing or verbally if the student cannot submit the request in person; limited to 180 days in any 12-month period; approved leave time does not count against attendance requirements (see the School Catalog).

I have read and understand the LPN Attendance and Satisfactory Academic Progress Policy.

  • Disclosures for Prospective Students and Employees

  • CAMPUS SECURITY INFORMATION
The school prepares an Annual Security Report to comply with the Jeanne Clery Disclosure of Campus Security Policy and Campus Crime Statistics Act (Clery Act). The report is prepared in cooperation with local law enforcement agencies and includes statistics for the previous three calendar years concerning reported crimes that occurred on campus, in certain off-campus buildings or property owned or controlled by the school, and on public property within or immediately adjacent to and accessible from the campus. It also includes institutional policies on campus security, sexual misconduct, and other matters. Nothing in the law permits the school to retaliate, intimidate, threaten, coerce, or otherwise discriminate against any individual with respect to the implementation of the Clery Act.

Enrolled students, faculty, and staff are notified of the report annually. Prospective students and employees may obtain a copy from the School Director or at cde.edu/disclosures.

    IMPROVEMENTS TO ACADEMIC PROGRAMS
The school reviews its academic programs regularly to ensure relevance to current employment requirements and market needs and may change, amend, or modify program offerings and schedules in accordance with New Jersey Board of Nursing and New Jersey Department of Labor and Workforce Development requirements. Questions about this process may be directed to the Director of Education.

    COPYRIGHT INFRINGEMENT
Unauthorized distribution of copyrighted material, including unauthorized peer-to-peer file sharing, may subject students to civil and criminal liabilities, including monetary damages, fines, and imprisonment. The school prohibits use of its computers and networks for unauthorized downloading, uploading, or storage of copyright-protected material. Disciplinary action, up to and including expulsion, will be taken against students who engage in unauthorized distribution of copyrighted materials using the school’s information technology systems.

    Student Demographic Information
Information about the composition of the student body is available on the College Navigator website (nces.ed.gov/collegenavigator), maintained by the U.S. Department of Education’s National Center for Education Statistics.

    Vaccinations
CDE Career Institute does not require institution-wide vaccinations for general enrollment. However, the Licensed Practical Nursing program requires documentation of specific immunizations and health screenings as a condition of placement at clinical sites, in accordance with clinical affiliation agreements and New Jersey Department of Health requirements. The requirements are listed on American DataBank’s Complio portal and are explained to students upon enrollment.

    LPN Admission Application

    Complete the form below.
  • Clinical Site Requirements & Student Agreement

  • This agreement sets out the mandatory clinical requirements that every LPN student must follow while participating in clinical experiences. Clinical education is a required, non-negotiable component of the LPN program and is governed by the CDE Career Institute School Catalog, the LPN Program Academic Standards Addendum, clinical facility rules, and applicable regulatory requirements, including those of the New Jersey Board of Nursing (N.J.A.C. 13:37).

Failure to comply with these requirements will result in immediate corrective and/or disciplinary action, which may include removal from the clinical site, loss of clinical hours, an Unsatisfactory clinical evaluation, suspension, clinical course failure, or dismissal from the LPN program.

  • 1.  ATTENDANCE & PUNCTUALITY

    • Clinical attendance is mandatory and strictly enforced. Students are required to attend every scheduled clinical day for the full assigned shift; clinical hours are a licensure requirement and cannot be waived.
    • Absence limit: a student may be absent for no more than two (2) clinical days per 15-week term. Absences beyond this limit will result in clinical course failure and may result in removal from the clinical site, administrative withdrawal, or dismissal from the LPN program. Partial-day absences (late arrival or early departure) are accumulated and counted toward this limit.
    • Students must arrive on time and remain for the entire assigned clinical shift. Late arrival, early departure, or leaving the site before the end of the shift are not permitted and are recorded as missed clinical time.
    • Any clinical absence (for example, illness or a verified emergency) must be reported before the start of the shift to both the clinical instructor and CDE administration. Advance notice does not excuse the absence and does not exempt it from the two (2)-day limit; every missed clinical day counts, regardless of the reason.
    • All missed clinical hours must be made up before graduation, without exception. Make-up is not guaranteed: it is scheduled only as instructor, clinical facility, and clinical site availability permit, and it does not waive the two (2)-day absence limit. A student who is unable to complete required make-up hours cannot satisfy the clinical course requirements and will not be eligible for graduation. There is no charge for make-up sessions.
    • A student who reaches two (2) clinical absences in a term must meet with the Director of Education within five (5) business days to develop a written make-up and remediation plan, and may not begin a new clinical rotation until the plan is in place. Continued non-compliance may result in course failure, removal from the clinical site, and administrative withdrawal.
  • 2.  CLINICAL CLEARANCE & HEALTH REQUIREMENTS

    • Before beginning clinical rotations, students must complete a criminal background check (N.J.A.C. 13:37-1.8(h)) and a drug screening, and must submit all required immunization and health records and current American Heart Association CPR/BLS certification.

    • American DataBank’s Complio portal: all clinical clearance requirements — including immunizations/vaccines, criminal background check, and drug screening — may be completed through American DataBank’s Complio portal. Portal access instructions, submission deadlines, and any associated costs are provided to students upon enrollment. Students remain responsible for completing every requirement by the stated deadline; a student who is not fully cleared may not begin or continue clinical rotations.

      • Students pay the clearance fee directly to American DataBank.
      • The clearance fee is not included in tuition.
      • Students may not begin clinicals until Complio reflects that they are fully cleared.
    • Clinical facilities may impose additional screening or health requirements. A student who does not clear a background check, drug screening, or facility requirement may be unable to complete clinical requirements and may be dismissed from the program.

    • Students have a continuing duty to notify the Director of Education immediately of any new criminal charge or conviction, or any change in health clearance, until the program is completed.

  • 3.  UNIFORM & APPEARANCE REQUIREMENTS

Students must adhere strictly to the clinical dress code at all times:

    • The official CDE Career Institute LPN uniform is required: clean, pressed, properly fitted scrubs in approved program colors.
    • The CDE-issued photo ID badge must be worn and visible at all times.
    • Closed-toe, non-slip, professional nursing shoes are required (no Crocs).
    • Hair must be neat and secured; facial hair must be clean and groomed.
    • Minimal jewelry: wedding band and small stud earrings only.
    • No artificial nails, gel nails, or nail polish.
    • Tattoos must be covered if required by the clinical site.
    • No excessive makeup, perfume, or cologne.

Students who report to clinical without the proper uniform or appearance may be denied entry to, or immediately removed from, the clinical site; the clock hours missed are recorded as an absence and count toward the two (2)-day limit, and the student is subject to disciplinary action.

  • 4.  PROFESSIONAL CONDUCT & BEHAVIOR

Students are expected to demonstrate professional behavior at all times, including:

    • Respectful communication with patients, staff, instructors, and peers.
    • Adherence to HIPAA and patient confidentiality requirements.
    • Compliance with all clinical site policies and procedures.
    • Use of professional language and demeanor.
    • Zero tolerance for harassment, disruptive behavior, or disrespect.

Any unprofessional conduct will result in immediate removal from the clinical site and may lead to suspension, clinical course failure, or dismissal from the program.

  • 5.  ASSIGNMENTS & CLINICAL PREPARATION

    • All clinical assignments must be completed and submitted by the assigned deadline.
    • Students must arrive clinically prepared, having reviewed patient information and required materials.
    • Failure to complete or submit required clinical assignments will result in an Unsatisfactory clinical evaluation for the day.
    • Clinical practicum courses are graded Pass or Unsatisfactory. A student who receives an Unsatisfactory clinical evaluation receives an Incomplete (I) for the clinical practicum and must remediate before continuing. Repeated failure to complete clinical assignments will result in course failure, and failure of two (2) or more nursing courses (theory or clinical) results in dismissal from the LPN program.
  • 6.  SAFETY & SCOPE OF PRACTICE

    • Students may perform only skills that have been taught and approved by the program and the clinical instructor.
    • All skills must be performed under appropriate supervision.
    • Any needlestick, blood or body-fluid exposure, injury, or patient incident must be reported immediately to the clinical instructor.
    • Unsafe practices or violations of scope of practice may result in immediate disciplinary action.
  • 7.  ELECTRONIC DEVICES

    • Personal electronic devices (including cell phones, smart watches, earbuds, and headphones) are strictly prohibited during clinical hours.
    • Use of electronic devices will result in immediate dismissal from the clinical site and disciplinary action.
  • 8.  DISCIPLINARY PROCESS & RELATED POLICIES

Time missed because of removal from a clinical site is recorded as an absence and counts toward the two (2)-day clinical absence limit. Disciplinary actions (warning, disciplinary probation, suspension, or dismissal) are administered under the Student Conduct Requirements in the School Catalog, and dismissal is confirmed in writing by the School Director. Students may use the Student Complaint and Grievance Procedure in the School Catalog. If a student withdraws or is dismissed, the Return of Title IV Funds Policy and the Cancellation and Refund Policy apply. Clinical attendance status is an academic status of the LPN program and does not by itself change a student’s federal financial aid eligibility, which is determined under the Satisfactory Academic Progress policy.

  • Information and Sharing

The Family Education Rights and Privacy Act of 1974 (FERPA)
  • WHAT IS FERPA?

The Family Educational Rights and Privacy Act of 1974, as amended, (commonly known as the Buckley Amendment) is a federal law which provides that schools will maintain the confidentiality of student education records. The law basically says that no one outside the institution shall have access to student's education records nor will the institution disclose any information from those records without the written consent of students. There are exceptions, of course, so that certain personnel within the institution may see the records, including persons in an emergency to protect the health or safety of students or other persons.

    WHAT IS CONSIDERED DIRECTORY INFORMATION?

In compliance with FERPA, the following statement reflects the school's policy: The following directory information may not be released by phone: A) Student's dates of attendance; B) date of graduation and degree or certificate earned. Other kinds of directory information, such as a student's address, telephone listing, program of study, awards received, and the most recent previous education agency or previous institution attended, will be released only in response to a written request.

The school reserves the right to refuse the above information if the reason for the request is not considered to be a sufficient need to know. Information regarding the student's record: Grades, courses, GPA, social security number and other personal information will not be released without the student's written consent.

However, the Act states that each student has the right to inform the school that any or all of the information is not to be released. No information will be released without consent of the student. This status is binding until such time that CDE Career Institute is notified in writing by the student to permit release of " directory information".

    WHAT ABOUT...?

At the postsecondary level, parents have no inherent rights to inspect a student's education record. The right to inspect is limited solely to the student. Records may be released to parents only under the following circumstances: (1) through the written consent of the student, (2) in compliance with a subpoena.
